Initial Consultation
Preparation
To prepare for the Initial Consultation, please provide:
- A completed questionnaire.
- A short video of your child.
- Please send the completed questionnaire and any available test results to gkraevaia@gmail.com. Use your full name in the subject line of the email.
All materials must be submitted no later than one week prior to the consultation to allow sufficient time for review. - QuestionnairePlease, download the questionnaire file.
Complete the questionnaire and rename the file as follows:
"Questionnaire - First Name Last Name" (use the child’s name and surname).
- VideoProviding a video helps me assess your child’s abilities in advance, saving valuable consultation time. This allows us to focus more on discussing specific concerns and finding solutions during the meeting.
Prepare a 5–10 minute video showcasing the child’s behavior, following the video requirements. The video should be recent and reflect the child’s current behavior. Upload it to a cloud service (e.g., Google Drive) with viewing permissions enabled, and share the link in the questionnaire or via email.
Video requirements
1️⃣ Total Video Length:
The video should be between 5–15 minutes. If you cannot capture everything in one session and are unable to edit videos, you may upload 2–3 shorter videos, totaling no more than 15 minutes.
2️⃣ Gross Motor Skills:
Show how the child walks, runs, jumps, and climbs stairs.
Capture activities such as dressing, drawing, or eating.
Show tasks the child can perform, such as stacking rings, solving puzzles, or matching pictures by criteria.
6️⃣ Emotional Engagement:
Record moments where you play with the child—such as playing ball, tag, or other active games that show their emotional reactions.
7️⃣ Close-Up of the Face:
In at least one part of the video, show the child’s face close-up in good lighting. I want to see their expressions, how they smile, react to your words, and how they articulate sounds or words.
What Not to Do
✗ Do not focus only on one activity, such as flashcards—it won’t be sufficient.
✗ Avoid filming the child from behind. I need to see their face, not their back.

- The child should wear light clothing—bulky clothes like coats and boots should be avoided.
- If the child has limited motor abilities, demonstrate what they can do.
Capture activities such as dressing, drawing, or eating.
- If the child is unable to perform these tasks, record any activities they can do with their hands.
Show tasks the child can perform, such as stacking rings, solving puzzles, or matching pictures by criteria.
- If the child works with an ABA therapist, recording these sessions is highly recommended.
- Ask the child to follow simple instructions, such as bringing a toy or placing it in a specific location, and record their response.
- Encourage the child to say a few words or sounds, depending on their ability.
- If the child is verbal, ask them to say something, answer questions, and demonstrate how they respond.
